This is a Consent Agreement Form; by signing and submitting this form, you keep complete copyrights of your work and you agree to authorize Two Girls and A Bible Inc. to use your testimony in any format, including photographs, videos, artwork (both digital and printed), direct quotes, and audio clips of programs and/or events, books, ebooks, blogs, and other printed or digital materials in any format to support the mission to "Go and Tell" the stories Jesus is writing on hearts today.
Works produced through Two Girls and a Bible, Inc. may be used for a variety of purposes, including but not limited to publications, promotional brochures, promotions, or showcases of programs on our websites, activities in local and national or international newspapers or programming, and other similar lawful purposes that include but are not limited to sellable material and gifts.
By signing and submitting this form, I consent and permit to allow Two Girls and a Bible the unlimited rights to use photos, videos, direct quotes, artwork, books, ebooks, manuscripts in both written and digital formats, and/or audio clips of me and/or my testimony and/or any work that is recorded by Two Girls and A Bible or submitted to Two Girls and A Bible Inc. through, but not limited to, website, social media accounts, and/or email.
Further, by signing this consent agreement and submitting this application form, I acknowledge that I understand and agree to the above request and conditions. I sign this form freely and without inducement.
